With this tool, users can build a binder of commonly used forms, and download or email forms that can be shared with schools and healthcare or service providers.

General The Guiding People Through Systems (GPS) website (“Site”) is an online resource for parents, family members and caregivers of children, youth and adults with disabilities and special health care needs. The Site helps organize and store important personal, care, medical and educational documents and resources for a user’s children/family members/guardians in one, convenient location. Information may be stored in an electronic, online binder or by downloading documents and resources on a personal computer. By using the Site, you agree to be bound to these GPS Terms and Conditions of Use. You agree to use GPS for the purpose of organizing and storing documents and resources solely for the purpose of care of your child(ren)/family member(s)/guardian(s). The Site is hosted by Cincinnati Children’s Hospital Medical Center (“CCHMC”). License Subject to your continued compliance with these GPS Terms and Conditions of Use, CCHMC hereby grants to you a non-exclusive, non-transferable license to use the Site for your own benefit and for the benefit of your child(ren)/family member(s)/guardian(s). You will not cause, attempt, or permit reverse-engineering, disassembly, or decompilation of the Site or otherwise attempt to derive the source code of the Site. CCHMC retains all rights, title and interest now existing or hereafter in existence in and to the Site, source codes, enhancements, modifications, derivative works, training materials, and other proprietary information, including, without limitation, all rights to patents, copyrights, trademarks, and trade secrets. You agree that you do not acquire any rights, express or implied, therein. You will not modify or amend the Site or create any derivative works or improvements thereto (all such modifications, amendments, improvements of derivative works collectively referred to as 'Modifications') without the prior written consent of CCHMC. Modifications, whether authorized or unauthorized, will be solely owned by CCHMC and shall be subject to the limited licenses and restrictions set forth in this GPS Terms and Conditions of Use. CCHMC may discontinue your access at any time for any reason upon at least thirty (30) days prior notice. You grant CCHMC a perpetual, non-exclusive, non-transferable right to access the data input into the Site for the purpose of administering and maintaining the Site. Security and Confidentiality CCHMC will protect the data entered into GPS by you from unauthorized third party access. Firewalls, passwords, encryption and audit trails are used to safeguard information stored in GPS. In no event will you deliberately attempt to access information for any account other than the one you have either established or been provided access. If any unauthorized access to GPS by you is discovered, CCHMC will terminate your access. immediately. You are responsible for maintaining the security of each GPS account you have established or been provided access to by: 1) changing your password on a regular basis, and 2) keeping your login ID and password confidential. You understand that no system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information. When you access your GPS account or GPS forms, documents and resources, information will be transmitted over a medium that is beyond CCHMC’s control. YOU HEREBY EXPRESSLY ASSUME THE SOLE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTINAL INTRUSION OR OF ANY DELAY, FAILURE, INTERRUPTION, OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USE OF GPS. Please be aware that when you download documents, forms and resources to your computer(s) or print copies, this information is no longer secure, and CCHMC is not responsible for the security of the information. Your GPS I.D. and Password User names (I.D.s) and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access the same document and resources you can access. It is your responsibility to prevent disclosure of your login and password and to change your password if you feel that your security has been compromised. You may change your password at any time. You understand and agree that CCHMC takes no responsibility for and disclaims any and all liability for damages arising from a breach of confidentiality resulting from your sharing or losing your password. If CCHMC discovers that you have inappropriately shared your password or that you may have misused or abused your access privileges in any way, access to your GPS account may be terminated immediately. Your Responsibilities You agree that GPS is a document and resources storage service offered as a convenience to you. You understand that you may not transmit any information or message that contains unlawful, threatening, fraudulent, libelous, defamatory, obscene or abusive information or language. GPS may not be used for any illegal or unethical activities. Hacking, attempted access or other forms of inappropriate access or attempted access to unauthorized areas or information on the Site will lead to termination of use and potential legal prosecution. Website Links GPS may offer links to related websites not managed by CCHMC. These website links are for informational purposes only. CCHMC does not endorse and has not verified the accuracy of the information in/on these websites. No Warranties THIS SITE AND THE CONTENT CONTAINED HEREIN ARE PROVIDED BY CCHMC ON AN 'AS IS' BASIS.
